Objective To identify and address challenges in implementing an advance care planning (ACP) programme using the ‘Let Me Decide’ (LMD) advance care directive (ACD) in long-term care settings. Design Pilot study assessing the feasibility of implementing the ‘Let Me Decide’ advance care planning (LMD-ACP) programme in three long-term care residential facilities, with ongoing feedback from staff to help refine the programme for use in the Irish setting. Setting Two long-term care nursing homes and one community hospital, with a total of 289 beds. Methods A two-phased approach was taken: (i) a staff education programme in ACP (and in delivering basic palliative care) and (ii) delivery of ACP by these staff, which included education of both residents and their families.
